I understand that FIT FOR UMRAH involves physical
activity, including but not limited to walking,
mobility exercises, balance exercises, strength
exercises, functional movements and other forms
of physical training.
1. Health Declaration
I confirm that the information I have provided in
this questionnaire is true and complete to the best
of my knowledge. I understand that it is my
responsibility to disclose any medical condition,
injury, medication, physical limitation or other
circumstance that may affect my ability to
participate safely.
I understand that this questionnaire and the
FIT FOR UMRAH programme do not constitute medical
advice, diagnosis or medical assessment. Where I
have a medical condition, symptoms, injury or
uncertainty about my ability to exercise safely,
I understand that I may be advised to obtain
clearance from a doctor or other qualified
healthcare professional before participating.
2. Acknowledgement of Inherent Risks
I understand that participation in physical exercise
involves inherent risks, including fatigue, muscle
soreness, strains, sprains, falls, dizziness and
other injuries or medical events that may occur
despite reasonable precautions.
I voluntarily choose to participate in FIT FOR UMRAH
with knowledge of these inherent risks and agree to
follow the safety instructions, exercise modifications
and directions provided by the programme coach.
I agree to stop exercising and immediately inform
the coach if I experience chest pain, unusual
shortness of breath, dizziness, faintness,
significant pain or any other concerning symptom.
3. Personal Responsibility
I understand that I am responsible for exercising
within my own capabilities and for informing the
coach if an activity feels unsafe or unsuitable
for me. I will not knowingly participate against
medical advice or conceal information that may
materially affect my safe participation.
4. Release and Limitation of Liability
To the fullest extent permitted by Singapore law,
I accept the ordinary and inherent risks associated
with my voluntary participation in the programme
and agree that Al Hilal Services LLP, OneOasis
Travel Pte Ltd and Coach Muhammad Isa Bin Mohyi
Hapipi, together with their respective officers,
employees, representatives and agents
(“Programme Parties”), will not be responsible
for loss, damage or injury arising solely from
such inherent risks or from my failure to follow
instructions, disclose relevant information,
exercise within my capabilities, or comply with
medical advice.
Nothing in this acknowledgement excludes or
restricts any liability which cannot lawfully
be excluded or restricted under Singapore law.
5. Indemnity
To the extent permitted by Singapore law, I agree
to indemnify the Programme Parties against
reasonable losses, claims, costs or liabilities
arising directly from my own wilful misconduct,
material failure to follow safety instructions,
or knowing failure to disclose relevant health
information, except to the extent that such loss,
injury or liability was caused by the negligence,
breach of duty or other wrongful act of a
Programme Party.
6. Emergency Assistance
In the event of an emergency, I authorise the
Programme Parties to contact my emergency contact
and, where reasonably necessary, to arrange or seek
emergency medical assistance on my behalf.
I understand that I remain responsible for medical
or related expenses incurred on my behalf, except
where applicable law provides otherwise.
7. Confirmation
By submitting this form, I confirm that I have
read and understood this acknowledgement, have had
the opportunity to ask questions where necessary,
and voluntarily agree to its terms.
